Open Case Status: za-OpenCase

This report has been specifically created for the CiN Census to allow practitioners to specify the status of cases that are open at the end of the CiN Census reporting period (31st March).

Valid status values have been defined by the DfE.

Referral and Information Record: zb- Referral

The Referral and Information Record gathers together the essential information about a child or young person. Where services are being provided, the Referral and Initial Information Record should be regularly updated to ensure essential information about a child or young person and family remains accurate and up to date.

There is an expectation that within one working day of a Referral being received there will be a decision about what response is required (Paragraph 3.8, Framework for the Assessment of Children in Need and their Families, 2000).

Initial Assessment: zc-Inassess

An initial assessment is defined as a brief assessment of each child referred to Social Services with a request for services to be provided. This should be undertaken within a maximum of 7 working days from the date of referral but could be very brief depending on the child’s circumstances. 

On completion of the Initial Assessment, a workflow message is generated to the responsible officer or post to read: “Please create the Initial Child’s Plan”. 
If the outcome of the Initial Assessment and/or the Core Assessment is Private Fostering Visit under Regulation 8, a workflow message will be generated for each outcome to notify the Social Worker. Message: “A private fostering visit is required in 6 weeks from <Assessment Date>.

In completing this initial assessment, if it is known that a Core Assessment is necessary to develop a fuller understanding of what is happening to a child and family in complex circumstances, Social Work staff should make a professional judgement about whether it is necessary to complete all sections before beginning a Core Assessment.

Record of Strategy Discussion: zj-CP1

A strategy discussion should be held whenever there is reasonable cause to suspect that a child is suffering or likely to suffer significant harm. This may include children who are already looked after, for example, when there is concern about abuse by carers.

This provides a record of the decisions of a strategy discussion/meeting. In some situations, it may be appropriate for a strategy meeting/discussion to consider all children/young people living in a household. Where this occurs, a separate record should be completed for each child/young person.

A Child Protection Plan can be generated at any time.

Record of Outcome of s47 Enquiries: zk-CP2

This form allows you to detail actions carried out following the strategy discussion(s), recorded on the Record of Strategy Discussion form, and summarise findings in terms of child’s developmental needs, including child’s safety; parenting capacity; and wider family and environmental factors.

The Record of Outcome of s47 Enquiries should be completed at the conclusion of the enquiries to record the decisions made and the evidence that informed these.

A Child Protection Plan can be generated at any time.

Initial Person Protection Conference Report: zl-CP3

The Initial Child Protection Conference Report provides a summary of the information gathered by social services in the course of the initial assessment, and the s47 Enquiry as part of the Core Assessment. For a child and/or family known previously to Social Services, it may also include existing information pre-dating this s47 Enquiry. Paragraph 5.61 in Working Together sets out the information to be included in this Report.

An Initial Child Protection Conference should be convened if, having completed s47 enquiries, it is concluded that a child may continue to suffer, or be at risk of suffering significant harm.

On completion of the Initial Child Protection Conference, a workflow message is generated to the responsible Child Protection officer or post to read: “Please create Outline Child Protection Plan”.

Core Assessment Record: zd-Coreass0-1

Core Assessment Record: Pre-birth to Child Aged 12 Months

The Core Assessment Record provides a framework for systematically recording the findings from the core assessment. Whilst the Assessment Record provides some guidance on the areas that should be covered in a core assessment, it is a tool and should not be used as a substitute for a professionally informed assessment process, analysis and judgement.

This provides a structured, in-depth assessment of the child’s needs where their circumstances are complex.

A Core Assessment should be completed within 35 working days of its commencement.

Child/Young Person’s Plan: zm-Cypplan

The Child’s Plan has been designed for use with all children in need living with  their families, including those for whom there are concerns that they are suffering or are likely to suffer significant harm. This plan may also be used with children and young people receiving short break care, in conjunction with Part One of the Care Plan.

This plan sets out the agency’s plan for a child/young person living with his/her family within the community. This plan should be completed after analysing the findings from a core assessment, and updated or changed as appropriate following a review.

Placement Information Record: zn-Placerec

The Placement Information Record details the arrangements for meeting a child or young person’s needs where responsibilities are divided between a number of people.

A Placement Information Record should be completed for all children and young people looked after away from home, including those receiving short break care. It should be completed by the time a child or young person is placed.

This may also be used for children and young people receiving regular day care services and support foster care which does not include overnight stays.

The Placement Information Record details the arrangements for meeting a child or young person’s needs where responsibilities are divided between a number of people.

Child/Young Person’s Care Plan: zo-Careplan

A Care Plan should be made before any child/young person is looked after, even if only for a few days. Wherever possible, the Care Plan should be completed before a child/young person is first looked after and in the case of an emergency admission, it should be completed, as soon as possible after a placement has been made.

The Care Plan is in two parts. Part One sets out the agency’s overall plan for the child/young person and Part Two details how the child/young person’s needs will be met, identifying what services and actions are required to respond to the child’s identified developmental needs during the period they are looked after. 

Part One can only be changed through a formal Statutory Child Care Review at which all key parties are present.

Part Two: Chair’s Report

This section of the review sets out a summary of the discussion and decisions for the child/young person’s record. It should be completed by the review chair at the end of the review process and a copy provided to all participants. The Child’s Plan should be changed or updated following this review meeting.

Part One: Social Worker’s Report

Details of Social Work interventions since Child’s name was placed on the Register or last Child Protection Review Conference.

Part Two: Chair’s Report

This section of the review sets out a summary of the discussion and decisions for the child/young person’s record. It should be completed by the Review Chair at the end of the child protection review and a copy provided to all participants, along with the minutes of the Child Protection Review Conference.

If the outcome of the Child Protection Review is that the child is no longer to be subject to a CP Plan, a workflow message is generated to the responsible officer or post to read: “Please end the Initial Child’s Plan”.

This section of the review sets out a summary of the discussion and decisions for the child/young person’s record. It should be completed by the Review Chair at the end of the review process and a copy provided to all participants.

Child/Young Person’s Adoption Plan: zw-Adopplan

The Adoption Plan substitutes for Part 2 of the child’s Care Plan once a decision has been made at a child care review that the plan for the child/young person is adoption.

The plan covers all the steps which need to be completed for a child to be adopted and allows councils to monitor compliance with the timescales set out in the National Adoption Standards. The plan should be updated and reviewed on a regular basis.

Pathway Plan: zx-Pathplan

Part One:  Assessment

A pathway plan must be completed for each young person who is eligible, as defined by the Children (Leaving Care Act) and for each relevant child who does not already have one. 

Arrangements, including the timetable, for completing the needs assessment required for the pathway plan should be discussed and agreed at the young person’s statutory review prior to their sixteenth birthday, and should be completed within three months of this date.

Young people should be actively involved in the assessment and planning processes. Young people should be encouraged and supported to complete the My Pathway Plan consultation paper as part of the process of completing the Pathway Plan.

Part Two: The Plan - Guidance for Inclusion in the Pathway Plan

  • Health and DevelopmentHealth and Development
    • Information about support necessary to lead a healthy lifestyle e.g. dietary needs.
    • Name and address of GP.
    • Details of any specialist treatment required and how the young person will obtain this.
    • Emotional and mental health needs with details of services necessary to meet these.

    • Summary of the young person’s educational attainment referring to their Personal Education Plan.
    • Programme for him/her to be maintained in order to access education or training opportunities - including the personal support that will be available and how other agencies will assist this objective.

    • Information about the young person's individual goals and ambitions for work.
  • Emotional and Behavioural DevelopmentEmotional and Behavioural Development
    • Young person’s ability to maintain positive relationships.
    • How the young person copes with difficulties and frustrations.
    • Involvement in offending.
  • IdentityIdentity
    • The plan should be based on a clear understanding how the young person defines their own identity.
    • It should clarify how the young person will be supported to sustain a positive image and maintain links with their heritage.
  • Family and Social RelationshipsFamily and Social Relationships
    • Information about the practical and emotional support their family will offer a young person to maintain family and other relationships.
    • Details about other significant people who may be able to assist the young person establish a more independent lifestyle.
  • Practical and other skills necessary for independent livingPractical and other skills necessary for independent living
    • Evaluation of the young person’s current practical, social and emotional skills.
    • Information about the skills that he/she may need to acquire to enhance his/her abilities to manage successfully in their own accommodation and any support necessary to develop these skills.

    • Summary of support necessary to enable the young person to develop budget management skills.
    • For relevant young people - information about the funding necessary to meet their basic needs including how they will draw their financial support; arrangements for funding other needs.
    • How to obtain funds in an emergency.
    • Bank account details.
    • Information about help that will be funded to enable young people to set up their own accommodation when they leave care.
    • Details of arrangements to ensure that a young person’s property and accommodation is covered by adequate insurance.
    • Arrangements to offer the young person independent financial advice if required.
  • AccommodationAccommodation
    • Information about where the young person will live from age 16.
    • Arrangements and timing for the young person to move to more independent accommodation based on an assessment of their practical and other skills.

Closure Record: zy-Closure

This report summarises the impact of actions and services provided to a child and family, and the evidence that supports the decision to close a case. This is used when it is planned to cease work with a child or young person and family.

It records a summary of interventions and actual outcomes from date of most recent Referral and an overall summary of progress achieved, reasons for successful outcomes or for non or partial achievement of planned outcomes.  

A Closure Record should be completed on all cases that progress beyond an Initial Assessment (ZC-Inassess).
