Item 3.2 Assessment outcome date <AssesmentOutcomeDate>

Date of which EHC plan was issued is reported when:

• They have an EHCP Assessment Involvement that met the criteria for inclusion in Module 3.
• They are reported as Y – It was decided an EHC plan would be issued in Item 3.1 Assessment outcome
• The Date Final Issued is populated.

In this case, the Date Final Issued is reported as the Assessment outcome date.

Date at which person was notified of decision not to issue a plan is reported when:

• They have an EHCP Assessment Involvement that met the criteria for inclusion in Module 3.
• They are reported as N – It was decided an EHC plan would not be issued in Item 3.1 Assessment outcome
• The Notified Date – No Following Assessment is populated.

In this case, the Notified Date – No Following Assessment is reported as the Assessment outcome date.
