Hearing Impairment

Access to Hearing Impairment records will be available as an additional link from the Involvement.

The link will only be made available under the following conditions:

  • The local Authority has the appropriate Hearing Impairment License
  • The appropriate User Group Processes are set
  • The Show Hearing Impairment flag is set to True against the Service Team
  • The user has access to the Service Team via Establishments and Posts
  • The user has accessed the Involvement details for a CSS Generic Involvement

Where the link is available and accessed you will be presented with an embedded Online Hearing Impairment page, but from within the v4 application.

The recording of a Hearing Impairment for a Student is available through an Involvement of Type ‘CSS Generic’, subject to access rights. However, whereas a ‘CSS Generic’ Involvement form can have none, one or many Subjects, Hearing Impairment can only be recorded against a ‘CSS Generic’ Involvement that has a single subject recorded.

The use of a CSS Generic Involvement through which to record details of a Student’s Hearing Impairment enables a Caseworker history for a Service Team using Hearing Impairment to be maintained. This removes the need for an existing Hearing Impairment Referral in v3 to be closed and a new Hearing Impairment Referral to be opened (and therefore all the Hearing Impairment detail to be re-input) when the Caseworker is changed.

Open an Involvement and click on Hearing Impairment in the Links panel to open the following tabs:

  • Validation ErrorsValidation Errors

    As you are recording Hearing Impairment tests you may receive Validation Errors. These will display as a pop-up to alert you. They show where the problem lies (Found In) with a Validation Message.

  • SummarySummary

    The Summary tab is a read-only summary of the Hearing Impairment, together with the graph from the latest Audiogram Test.

    If at least one Audiogram Test exists then the Summary tab is the first tab to be displayed when accessing Hearing Impairment. If Hearing Impairment details have not yet been recorded then the Details tab will open.

    If more than one Audiogram Test exists then the tab shows the graph for the latest Audiogram Test by default. To display previous Audiogram Tests select a different date and/or time.

    At present only Audiogram Test Results for Right Ear (Unaided) and (Left Ear Unaided) are plotted on the Audiogram graph.

    Hearing Loss TypeHearing Loss Type Table_ID 0733Loss Type QualifierLoss Type Qualifier Table_ID 0734, Cause, and Loss Class (Left and Right) are recorded on the Details tab. (Loss Class (Left and Right) can be changed here).

    Loss Class (Left) and Loss Class (Right) and Audiogram Tests dates are automatically updated from the Tests tab.

    The Summary graph shows the Hearing Level in Decibels (dB) and the Frequency in Hertz (Hz). It displays data for Right Ear and Left Ear. Hover over a point (0 or X) on the graph to view the individual Frequency and Hearing Level.

    It is possible to Print any Audiogram displayed on the Summary tab.

  • Details
    The Details tab will open first if Hearing Impairment details have not yet been recorded. If Hearing Impairment details are available then the Summary tab will open.
    This tab allows you to maintain the details of a Hearing Impairment for a Student. You can record Hearing Loss Details, Additional Details, Hearing Aid Details and Loaned Equipment.
    • Hearing Loss DetailsHearing Loss Details

      Hearing Loss TypeHearing Loss Type and Loss Type QualifierLoss Type Qualifier will display a warning  that data is missing. These are not mandatory fields as it may be necessary to enter a test before this information is known. The icon will disappear once data is entered.

      Enter Cause information; this is a free text field.

      Loss Class (L) and Loss Class (R) are automatically filled from the Tests tab, but you can select manually from the drop-down 

      Click Enable to Auto-update the Loss Classes. This can only be done if Audiogram Results are available. If changes are made manually to the Loss Classes then Enable is turned off.

      Save the details.

    • Additional DetailsAdditional Details

      The Additional Details sub tab allows you to record information for two Cochlear Implants. Check to record a Cochlear Implant, use the calendar to select the Date of Implant, use the drop-downs to select which Ear and the Type of ImplantType of Implant.  

      You can also record information for two Bone Anchored hearing aids. Check if a Bone Anchored hearing aid is in use, use the calendar to set Hearing Aid Date, use the drop-down to record which Ear.

      Enter Date of Diagnosis and Age at DiagnosisAge at Diagnosis will be calculated automatically. Enter Date of Onset and Age at OnsetAge at Onset will be calculated automatically.

      Click the browse to open the Base Enquiry to select a Hospital. Use drop-down to select Health DistrictHealth District.

      The ENT Consultant allows you to select from people added to that Role table. The Consultant’s name will be added to the Contacts for the student.

      Use the drop-down to select Communication ApproachCommunication Approach e.g. Sign Language.

      Values for the Cochlear Implant migrated from v3 are displayed in the Cochlear Implant 1 field with an Ear of ‘Unspecified’.
      Values for the Bone Anchored Hearing Aid migrated from v3 are displayed in the Bone Anchored 1 field with an Ear of ‘Unspecified’.
      The Hospital data field is now populated with Base Type ‘Hospital’, rather than Agency as in v3.

      Save the details.

    • Hearing Aid DetailsHearing Aid Details

      This section allows you to record the Model and Serial No. of any Hearing Aids for both Left and Right ears and also any Spares. Use the calendar to set Issue Dates and Cease Dates, if appropriate. Use the drop-down to select a ProviderProvider, if required.

      Radio Aid Information (HI)

      This section allows you to record the Frequency of the Radio Aid, the AIS (L) (Auditory Input Signal Left) and the AIS (R) (Auditory Input Signal Right) in free text fields.

      Ear Mould Details (HI)

      If the hearing aid is moulded and fits into the ear you can record the AdaptationsAdaptations, the MaterialMaterial and the TypeType of Ear Mould for each ear.

      Save the details.

    • Loaned EquipmentLoaned Equipment

      Loaned Equipment is a read-only tab. The Equipment Details are set up in Focus | Equipment Inventory. Equipment Category must be matched to an External Code of HEAR or RAD and the Loan must be linked to the Person in the Involvement.

      All the Hearing and/or Radio Aid Equipment on loan to the student is displayed in this section. A Radio Aid can consist of two separate Equipment Items, both of which could be loaned out at the same time. Use the arrows to open and close the lists.

      To help with your search the Filter functionality is available.

        Equipment Loans can be grouped by Equipment Category if required. Select the column to be Grouped by: by dragging and dropping a column header. To remove the Grouped by: Header click on the ‘x’ on the button.

    • MemoMemo

      The Memo tab allows you to record any information related to the Hearing Impairment record.

      The memo can be formatted as follows:

      • Font – size, style, case, colour
      • Paragraph – alignment, bullets, numbering, text wrapping
      • Insert – symbols
      • Tools – undo/redo, find/replace, spell check
      • Toolbar – show or hide
      • Print Layout – layout or draft
      • Zoom – set magnification level or drag in and out
      • Print – click to print memo

      There are two Memo sub-tabs available to record relevant information for Audiogram Results and ERA Results.

      Save the details.

    • Tests
      The Tests tab allows you to record Tests of different categories. It is currently possible to record Tests for Categories of Audiogram and ERA (Evoked Response Audiometry).

      It is anticipated that additional Test Categories will be available in future releases.

      • Audiogram ResultsAudiogram Results

        The Audiogram Tests section provides the following:

        • Ability to log Test Results without having to specify Hearing Loss Type and Loss Type Qualifier first (as these values may not be known at the time that the Test results are recorded)
        • Automatic calculation of Average Hearing Loss (all Test Series) as values are entered
        • Automatic calculation of Loss Class for both Right Ear Unaided and Left Ear Unaided as values are entered
        • Increased range of Frequencies against which Test results can be recorded
        • Ability to choose the Test Series (e.g. Right Ear Unaided, Aided Threshold) required for an individual Test
        • Increased range of Test Series that can be chosen
        • Ability to record a ‘No Response (NR)’ against an individual Frequency

        Click New to Add Audiogram Results. Category and Test Date are mandatory fields.

        If there are already a number of records the Filter functionality is available.

        To remove existing Audiogram Results highlight the record and Delete. You will be asked for confirmation.

        To enter Test Type select from the dropdown.

        Test Location is a free text field.

        Click the browse to open the Person Search to record Tested By.

        Add a test from the list.

        Click in a box under each Frequency to record the results (valid value is between -10 and 130 or NR or None).

        Average Hearing Loss is automatically calculated from the following Frequencies: 250dB, 500dB, 1kdB, 2kdB and 4kdB.

        Loss ClassLoss Class is automatically calculated.

        As you are recording your results they will display on the graph showing Hearing Level in Decibels (dB) and Frequency in Hertz (Hz).

        At present only Audiogram Test Results for Right Ear (Unaided) and (Left Ear Unaided) are plotted on the Audiogram graph.

        When you Save the test results the graph will ‘disappear’ to the Summary tab.

        A warning icon will appear if the test result for either Left or Right Ear Unaided is missing. A valid Audiogram requires results from both Left and Right Ear Unaided tests.

        A Memo sub-tab is available to record any relevant Audiogram Results information.

      • ERA ResultsERA Results

        You can record tests for ERA (Evoked Response Audiometry) as a separate Category.

        Click New to Add ERA Results. Category and Test Date are mandatory fields.

        If there are already a number of records the Filter functionality is available.

        To remove existing ERA Results highlight the record and Delete. You will be asked for confirmation.

        Enter the results for both Right and Left Ear (valid value is between -10 and 130 or NR or None.

        Save the details.

        A Memo sub-tab is available to record any relevant Evoked Response Audiometry (ERA) Results information.

    • Legacy TestsLegacy Tests

      Any Hearing Test results entered in v3 are displayed in the Legacy Tests tab upon migration to v4. This data is read-only and ensures that historical test information recorded in v3 for Audiogram Results and/or ERA (Evoked Response Audiometry) Results remains available.

      Highlight the Legacy Test required and the Audiogram/ERA Results will be displayed below.

      Existing Memos can be viewed and printed from the Memo sub-tab.

    • UdfsUdfs

      Within Tools | Administration | UDF Management, Hearing Impairment is available within the Entity Selector and you can create and assign all UDF types.

      Hearing Impairment is only available in v4 as a link from Involvement Details and they are embedded online pages.

      It should be noted that, although the UDF Management functionality allows you to drag and drop a specified UDF to a specific panel and preview the field, this will only apply to v4 forms. The tabs for Hearing Impairment are online pages and therefore UDFs are displayed in a specific UDF tab rather than a specified panel. However the panel specified in UDF Management will be identified in the UDF tab. It will also list the panels which have UDFs assigned to them in the same order as other panels in One v4 and the UDFs will be listed in the order in which they appear within a v4 panel.

      To enter information highlight the required UDF and enter relevant data.

      Save the details.

      If UDFs have not been assigned you will receive a message “No UDFs have been designed”.

    • ReportsReports

      Click on the Hearing Impairment Summary in the Reports section on the Links panel to open the Report Viewer. The report will contain the data relating to the Student’s Hearing Impairment.

      You can print the Summary Report from the Report Viewer.

    Visual Impairment

    Access to Visual Impairment records will be available as an additional link from the Involvement.

    The link will only be made available under the following conditions:

    • The local Authority has the appropriate Visual Impairment License
    • The appropriate User Group Processes are set
    • The Show Visual Impairment flag is set to True against the Service Team
    • The user has access to the Service Team via Establishments and Posts
    • The user has accessed the Involvement details for a CSS Generic Involvement

    • Vision Issues/Conditions
      The system uses ‘expanders’  to record one or more Vision Issues and one or more Eye Conditions for Visual Impairment. This allows all selected Vision Issues and all selected Eye Conditions to be displayed on the screen.
      A free text field is available to record the Cause.
      Save the details.
      • Vision IssuesVision Issues

        Click on the expander to Choose Vision Issues. Click Plus to Add the Available Vision IssuesAvailable Vision Issues to the Selected Vision Issues.

        Click X to Remove Selected Vision Issues back to the Available Vision Issues.

        Click on the icon to close Choose Vision Issues.

        The Filter functionality is available on the columns.

      • Eye ConditionsEye Conditions

        Eye Condition is not mandatory, but a warning icon  is displayed on the Selected Eye Conditions to indicate that this data is missing. This is to allow the results of one or more Vision Tests to be recorded immediately even though the Eye Condition may not yet be known.

        Click on the expander to Choose Eye Condition. Click Plus to Add the Available Eye ConditionsAvailable Eye Conditions to the Selected Eye Conditions.

        Click X to Remove Selected Eye Conditions back to the Available Eye Conditions.

        Click on the icon to close Choose Eye Condition.

        The Filter functionality is available on the columns.

    • Details
      The Visual Impairment Details tab allows the recording of further details of the Student’s Visual Impairment in the following areas: When the details have been entered  Save the record.
      • DiagnosisDiagnosis

        Use the calendar to set the Date of Diagnosis. The Age at Diagnosis, in Years and Months, is calculated automatically based on the date entered.

        Use the calendar to set the Date of Onset. The Age at Onset, in Years and Months, is calculated automatically based on the date entered.

      • Additional DetailsAdditional Details

        Select RegistrationRegistration from the drop-down.

        Enter the Registration No.

        Select Health DistrictHealth District from the drop-down.

        Select Hospital from the browse. (Base Type is ‘Hospital’, previously ‘Agency’. 

        Select the checkbox if Mobility Required and enter the Mobility Level into the free text field.

        Select Mobility Officer from the drop-down. Click to show Person Details.

        Select Ophthalmologist from the drop-down. Click to show Person Details.

      • Visual AidsVisual Aids

        Select Yes or No from the drop-down if Spectacles are used or not.

        Use Tick or Cross to record if the student uses any additional visual Aids e.g. Contact Lens, Braille, Moon or Touch Typing.

      • Other Low Vision Aids MemoOther Low Vision Aids Memo

        This Memo is available to record details related to any Other Low Vision Aids.

    • Eye Tests
      The Eye Tests tab allows you to record one or more Eye Tests against the Visual Impairment for the subject. The tab opens with the Eye Test Details Summary.
      The details of the Eye Test are grouped into sub-tabs to simplify the process of entering data and to enable you to navigate around the various components of the Test: When the details have been entered Save the record.
      • Eye Test Details SummaryEye Test Details Summary

        This gives a Summary of the Eye Test showing Test Date, Test Location and Tested By.

        Click New to set up a new Eye Test. Test Date is mandatory. Test Date, Test Location and Tested By are editable fields. Use Person Search to record Tested By.

        The Filter functionality is available on the columns.

      • Vision DetailsVision Details

        This gives a Summary of the Eye Test showing Test Date, Test Location and Tested By.

        Click New to set up a new Eye Test. Test Date is mandatory (). Test Date, Test Location and Tested By are editable fields. Use Person Search to record Tested By.

        The  Filter functionality is available on the columns.

        The Vision Details sub-tab records information about Near Vision, Distance Vision, Colour Vision Details and Field Difficulty.

        Near Vision

        Select Test TypeTest Type from the drop-down.
        Record Near at …..cm for Left Eye, Right Eye or Both Eyes.

        Distance Vision

        Select Test TypeTest Type from the drop-down.
        Record if the test was carried out With Glasses or Without Glasses for Left Eye, Right Eye or Both Eyes.

        Colour Vision Details

        Select Test TypeTest Type from the drop-down.
        Use  or  to record if the student has Colour Difficulty with Red/Green, Blue/Yellow or All Colours.

        Field Difficulty

        Use Tick or Cross to record if the student has Field Difficulty with Upper Left, Upper Right, Lower Left, Lower Right, Central or Depth Perception Difficulty.

      • Visual Attention/Light DifficultyVisual Attention/Light Difficulty

        Visual Attention

        Visual Attention Difficulties may be recorded. Use the free text fields to record Loses attention beyond, Preferred object colours and Preferred background colours.

        Use  or  to record Visual crowding difficulty, Hearing difficulty, Mainly attends to movement, Physical difficulty and Epilepsy.

        Light Difficulty

        If sight is affected by Light Difficulties they can be recorded here. Use the free text fields to record Best print colour and Best paper colour.

        Use Tick or Cross to record if the student has Low light difficulty, has Sensitivity to light and glare or has Difficulty adjusting to change in light level.

      • Visual Pursuit/LetteringVisual Pursuit/Lettering

        Visual Pursuit Difficulty

        Use Tick or Cross to record Visual Pursuit Difficulties as follows:

        Left > Centre Centre > Right Left > Right
        Right > Centre Centre > Left Right > Left
        Top > Centre Centre > Bottom Top > Bottom
        Bottom > Centre Centre > Top Bottom > Top

        Lettering

        Preferred and Threshold reading parameters may be entered indicating the Smallest Lettering No. and Distance Lettering No. (Font size).

      • MemoMemo

        This sub-tab allows any Memo information related to the Eye Test for the Visual Impairment to be recorded.

    Filters and SQL Mail Merge

    • Filter FunctionalityFilter Functionality

      The  Filter functionality is available where data grids are used to display summary lists of data. You can filter the browse list by any of the columns displayed.

      • Click the filter icon to open the filter
      • Click the checkbox to Select All
      • You can select any number of the data items
      • Use the drop-down to Show rows with value that –  Is equal to/Is not equal to/Starts with/Contains/Ends with
      • Enter the required value
      • Select the checkbox to Match case (not for dates or numbers)
      • Use the drop-down to set value at and – Is equal to/Is not equal to/Starts with/Contains/Ends with (if required)
      • Enter the required value (if ‘and’ was selected)
      • Select the checkbox to Match case (if ‘and’ was selected)
      • Click the square Filter icon when the values have been chosen
      • Use the Clear Filter icon to select different items or change values
      • Click away from the filter to return to the browse
      • The orange filter icon lets you know that a filter is ‘on’
    • SQL Mail MergeSQL Mail Merge

      Clicking the SQL Mail Merge button opens the SQL Mail Merge dialog for the current record, showing the name of the person logged on (User Name) and the area where the Mail Merge is being created (Entity Name).
